Incident Narrative

An employee was performing a tool change in an eight-spindle chucking machine. After completing the other seven spindles, he rotated the machine to the eighth spindle and pushed the stop button. The employee's left middle finger contacted a spindle before it had come to a complete stop resulting in a fingertip amputation.
