Incident Narrative

An employee was standing on a 10-foot aluminum step ladder reaching through a ceiling hatch during maintenance on a unit. After placing his tools through the hatch he lost balance and fell to the floor. The employee sustained seven broken ribs, a punctured lung, and a spleen injury.
