Incident Narrative

An employee was assisting in the operation of a pallet inverter. The employee noticed wood debris near the roller. As the employee was removing the wood, the inverter actuated and caught the employee's hand/arm resulting in a fractured arm and a laceration from the fingers to the forearm.
