Incident Narrative

An employee was roping an elevator car and lowering the ropes to the counterweight. Several feet of rope slack on the machine room floor needed to be released from the hoisting rope. When removing the wire rope clamps, the rope slid through the remaining clamp. The rope gained momentum and struck the employee on the right forearm and fractured it.
