Incident Narrative

Employees were working to re-line sewer lines with a fiberglass liner. The employees had cut out a piece of fiberglass liner and were pulling it up using a forklift and rope when the piece caught on the manhole rim. The rope was severed and the piece fell and struck an employee, who was in the manhole, on the left foot amputating the middle toe.
