Incident Narrative

An employee was changing a tooth on the bucket of an excavator. He was using a hammer and chisel to remove the tooth when a piece of the tooth fragmented and struck him on the left wrist and in the stomach. The employee sustained an internal injury to the stomach resulting in surgery.
