Incident Narrative
An employee was stepping from an interstitial concrete landing 10 feet above the floor onto a 16-foot fiberglass A-frame ladder. He had one foot on the interstitial landing and one foot on the ladder when the ladder tipped sideways and he fell 10 feet to floor below. He fractured his left heel bone and left L4 vertebra.
