Incident Narrative
On June 25, 2019, an employee was repairing a machine that moves pipes (36 inches in diameter, 1/2-inch thick, and 40 feet long) from a set position to a skid position. He was welding on a hydraulic cylinder clevis that holds the cylinder to the machine. When he moved the machine arm, it triggered the automated car to deliver a pipe to the machine. Seeing that a pipe was coming toward him, he jumped out of the way and landed near a skid pipe. He fractured his left foot and was hospitalized.
