An employee was removing the inside steel pieces of a 91-inch by 91-inch by 6-foot concrete form, breaking the form down. The floor piece of the form (weighing more than 1,000 pounds) fell onto the employee's right side, lacerating his liver and injuring his right kidney and spleen.
