Incident Narrative
On June 14, 2019, an employee was dumping approximately 3 tons of asphalt that was left inside the bed of a dump truck from the night before. The employee raised the dump truck and then exited the truck. While the employee was standing at the rear of the truck, the asphalt broke loose and fell onto the employee, pinning the employee between the pavement and the tailgate of the dump truck. The employee suffered burns, lacerations, broken bones, and the amputation of toes.
