Incident Narrative

An employee was removing a plywood blockout from inside a concrete box culvert segment. As the employee was sitting on the lip of the culvert and trying to grab a ladder, he fell off the culvert onto steel plate decking 3-4 below. He suffered a broken left tibia and was hospitalized.
