Incident Narrative

Two employees were tightening a belt on a conveyor while standing on a platform on a flatbed truck. The platform slipped down off the truck and both employees fell off the truck bed. The first employee sustained right-side stomach abrasions. The second employee fractured four ribs and was hospitalized.
