Incident Narrative

An employee was watching boxed product enter the tape machine when his shirt was grabbed by the machine, pulling the employee's arm into the equipment. The flight arm retracted and pinched the employee's wrist between the flight arm and the frame of the machine. The employee suffered a broken wrist that required surgery.
