Incident Narrative

On 5/7/2019, an employee was working on the ground level collecting falling brush from a tree and disposing of it in a wood chipper. A second employee was performing tree pruning and utility line clearance operations in a utility truck bucket approximately 45 feet high. The second employee began to cut a tree lead, approximately 15 feet in length, with a chainsaw. The tree lead then fell and struck the first employee's face, causing nose and facial lacerations, fractures, and contusions.
