Incident Narrative

An employee had been burning off old cloth that was on tongs so it could be replaced. To cool off the tongs, the employee accessed the water coolant stream on the upsetter forging machine. The employee slipped and stepped on the pedal that engaged the machine and half of his left pinky finger was amputated.
