Incident Narrative

An employee was about to climb down from a scaffold when he fell onto a concrete slab 6 feet below. He landed on his shoulder and head, his hard hat cracking on the ground; he suffered bleeding to the left ear and a concussion, as well as bruising to the left shoulder. He was hospitalized.
