Incident Narrative

An employee was helping to move a piece of casing off a guide wire from a rig. The casing was on a pipe rack. As the pipe was pushed to move it out of the guide wire, the pipe rolled and caught his right hand, pinching his middle finger and resulting in partial amputation of the fingertip.
