Incident Narrative

An employee was standing on a disassembled section of scaffolding frame, which was lying flat on the ground connected to a second frame. The second frame was hit with a hammer to dislodge it from the first, and the employee lost his balance and fell backwards. His right jaw and head area struck an I-beam and his jaw was fractured.
