Incident Narrative
An employee was transporting a box placed on the seat beside him on the forklift. The box fell while the forklift was in motion. The employee reached down to retrieve it and lost control of the forklift while going down a ramp. When the employee tried to regain control, he was ejected from the forklift and injured his hip.
