Incident Narrative

On 4/02/2019, an employee was approximately 6 feet off the ground on an 8-foot ladder, hand-adjusting a wire on ducting/sheet metal that was being hung. He became unbalanced and fell to the concrete floor, fracturing his left upper arm and requiring surgery.
