Incident Narrative

A transfer vessel was about to be pressurized to clear a blockage; an employee was standing in front of the discharge piping, holding a sheet of plywood to prevent material spillage. Air suddenly released from the discharge piping at 70 psi, forcing the plywood into the employee's face and throwing him about 15 feet. He suffered a fractured orbital socket, a laceration above the eyebrow, and a subdural hematoma.
