Incident Narrative

An employee was operating a motorized pallet jack. He picked up two pallets with the jack; the top pallet shifted, causing the bottom pallet to move as well. This caused the pallet jack to suddenly lower. It struck the employee's right shin and dropped onto his left foot. He was hospitalized for surgery on his foot.
