An employee was bending a piece of 1-inch-thick, 6-inch-wide, 6-foot-long flat steel in a press fitted with a four-way die. One of the fins on the die broke, allowing the steel to fall and strike the employee's right leg above the knee. The employee suffered a contusion and swelling and was hospitalized, requiring surgery.
