Incident Narrative
An employee was repairing a motorized boom conveyor used to load trailers. The employee used an open end wrench as a pry tool in an attempt to correct the tracking of the conveyor belt near the end pulley. When the unit was actuated to start to the re-track process, the wrench moved and pinched the employee's right pinky finger against a hard metal surface, amputating the finger from the first joint.
