Incident Narrative

An employee was serving as a lookout for a forklift operator who was picking up steel beams to move them on the job site. The forklift arm caught something on the ground and caused the load to bounce. When the load bounced, the beams toppled over. The employee was then pinned between a steel beam and a vehicle, suffering a cut to the upper left leg and bruising on the upper right leg.
