Incident Narrative

An employee was moving the floor scrubber to an area of the warehouse to drain the water reservoir. He opened the door and proceeded to move the floor scrubber towards the door. The scrubber fell out of the door and the employee jumped off of the scrubber and landed on his right elbow and side causing a right elbow fracture and bruising on the right side of his body.
