Incident Narrative

An employee was doing preventative maintenance on a forklift and was checking the fuel regulator on the unit. After cleaning the filter, the employee popped the bolts off the cover of the regulator and the propane blew up in his face. He suffered chemical burns to the left side of his face and both eyes.
