Incident Narrative
An employee was on an extension ladder, using a metal crowbar to remove shingles from near a second-story window on an outside detached garage. The ladder slipped from under him and he fell, landing on the black asphalt driveway in front of the garage. He suffered an injury to his back and was hospitalized.
