Incident Narrative

An employee on a 6-foot ladder, cleaning an external dryer vent that was about 11.5 feet above a patio. The ladder tipped over and the employee fell; his head hit the patio furniture and then the concrete patio itself, and he briefly lost consciousness. He was hospitalized.
