Incident Narrative
An employee entered a trench (about 7 feet deep and 7 to 8 feet wide) to install a fitting on a sewer main at the bottom. The trench's one vertical wall collapsed onto the employee, who suffered four broken ribs, a collapsed lung, and a broken shoulder blade.
