An employee was climbing a straight ladder that was secured to a boiler. On the third or fourth step of the ladder, the employee's foot slipped causing him to fall backwards and hit a fire extinguisher that was on the wall before falling to the concrete ground. The employee was hospitalized due to an injury to his twelfth vertebrae.
