Incident Narrative

An employee was operating a block chopper. A frozen block of product was not feeding properly. While the employee was adjusting it with a meat hook, the chopper grabbed the hook and the employee's right little finger was pinched between the hook and the chopper's feed shoot. The fingertip was amputated.
