Incident Narrative
An employee was standing on the back side of an 8-foot fiberglass portable ladder at the lower brace and handing a piece of ductwork up to a second employee who was standing on the 3rd or 4th rung on the front side of the ladder. The ladder collapsed and both employees fell to the ground. The ladder and ductwork landed on the first employee, who was hospitalized with a fractured lower left leg.
