Incident Narrative

At about 8:15 a.m., on January 18, 2019, two employees were trying to clear jammed lumber in a saw. While one of the employees was holding up a lumber indexing guide, the saw switched to reverse feed. A 2-inch by 4-inch piece of lumber caught the bottom of the guide, moving it forward, and pinched the employee's index and little fingers. The tip of the little finger was amputated.
