Incident Narrative

An employee was working on top of a mezzanine. He was getting ready to come down from the mezzanine floor when he fell approximately 12 feet from a fixed ladder to the lower level, suffering forehead and kneecap injuries that required stitches, a concussion, a swollen right eye, partial hearing loss in the right ear, and four broken ribs.
