Incident Narrative
An employee was checking 112" corrugated board for moisture coverage/dry spots to ensure that starch content was correct. While the employee's hand was on a fixed bar, the machine was turned off and another, pneumatically operated bar moved to its off position, pinching the tips of the employee's right index, middle, and ring fingers against the bar the employee was gripping. The employee suffered amputations to the fingertips.
