Incident Narrative

Two employees were unloading a slab of granite weighing about 1,000 pounds from a fifth-wheel trailer to an A-frame on the ground, using a clamp attached to a rented forklift. The A-frame shifted and the slab tipped over. One of the employees suffered a broken tibia and fibula, requiring surgical repair and pins; the other suffered several broken bones in the leg, as well as a possible broken hip and shoulder.
