Incident Narrative
An employee was standing and talking to another employee. He had his hand on the platform of a double impact or divine grinder. He stepped on the foot pedal which actuated the machine and his right hand was pulled in resulting in amputation of the pinky, ring and middle fingers as well as the index fingertip.
