Incident Narrative

An employee was driving a forklift onto a semi-trailer positioned at a loading dock at the employer's freight terminal. The semi-truck pulled away from the loading dock as the forklift was entering the trailer. The forklift fell to the ground between the back of the trailer and the loading dock and the forklift operator sustained fractured vertebrae.
