Incident Narrative

An employee was guiding a rod into the tongs. Once it was placed in the tongs, the wind pushed the rod toward him. To prevent the thread from stripping, he slightly pushed the rod away from himself. Then the rod started rotating clockwise and the employee's glove became caught around the rod. His right hand fingers were pushed back, dislocating two fingers.
