Incident Narrative
An employee was working from a ladder on the second floor of a residential house. There was a hole in the floor approximately 3 inches in diameter in the area where the ladder was set up. The ladder shifted to the side and the employee fell 4 feet to the wooden floor. The employee sustained a head injury and was hospitalized.
