Incident Narrative

On December 9, 2018, at approximately 11:00 AM, a maintenance employee of Toledo Elevator Company responded to a call to service a downed elevator. The employee tripped and fell down three flights of stairs. He was found at the bottom of a stairwell near the elevator shaft with multiple head lacerations, partial loss of an ear, and a fractured C-5 vertebra. The employee was hospitalized.
