Incident Narrative
Two employees used lock out/tag out (LOTO) procedures to change carrier chains. The chains became stuck. When one employee tried to release the chains, stored pneumatic energy was released catching the employee's right ring finger and crushing it to the first knuckle resulting in a partial amputation.
