Incident Narrative

An employee had been installing an approximately 52-foot long hydraulic jack into a PVC pipe in the ground. When there was about 5 feet of jack remaining, a pipe clamp broke causing the jack assembly to fall vertically. There was an approximately 2-foot square piece of metal plate attached to the top of the jack assembly. The employee's right arm was caught between the jack assembly's metal plate and the PVC pipe. The employee sustained crushing injuries including multiple fractures and the amputation of the arm from the elbow down.
