Incident Narrative

An batch operator was loading beans from a grain bin into a screw conveyor system (auger). The grate which sits above the conveyor and allows the product to flow into the auger became displaced exposing the auger. The auger blade caught his boot and pulled it into the conveyor system causing the amputation of the operator's big toe.
