Incident Narrative
An employee was spotting for a forklift driver while it was moving 20-foot long I-beams. The employee walked into a gap about 3 feet wide, between the stacks of I-beams the operator was lifting. As the operator was raising the load of steel with a combi-lift, the front stack started to tilt forward. The operator quickly lowered the steel to keep the front stack from toppling over onto the injured employee, but the front stack had passed the tipping point and the steel fell onto the employee's left leg, breaking it.
