Incident Narrative

An employee was dismantling scaffold that was three frames high and had planking that was three planks wide. As he was dismantling the ladder portion, the ladder tipped forward and he lost his balance and fell 18 feet to the ground, suffering a left heel fracture, a right ankle fracture and compression of numbers 1, 2 and 3 vertebrae.
