An employee was removing the guard from a driven roller assembly (to see why the roller was not operating) when his right index finger was caught between the drive belt and the pulley. The finger was amputated just below the nail line; he also suffered a small laceration to the right middle finger. The machine was not locked out at the time of the incident.
