Incident Narrative

A maintenance employee was trying to remove a choker sling on a pulley when the choker became caught and the pulley counterweight fell, causing the employee's foot to slip off of the equipment frame and become caught between the frame and the pulley. The employee suffered multiple ankle fractures.
