Incident Narrative

An employee had just finished rigging a bundle of channel iron together to be lifted to the roof of a building. When the load began to lift, the employee was stepping back from the load to grab the tagline when he reached out and lightly placed his left hand against the load to steady it. At that time, the bundle of channel iron rolled inward, catching his left glove and pulling his hand in between two pieces of iron. He was able to pull his hand back immediately, but his left middle fingertip was crushed, resulting in an amputation (without bone loss) of the distal tip of his left middle finger.
